Whether you’re purchasing customised supports with individualised funding, or you’re accessing one of the broad range of mainstream services we currently offer, our first step is to listen to you and learn about your individual situation.

We’ll take the time to find out about your interests and goals, develop an understanding of needs, and discover what wellbeing means to you.

Plan

Once we have a thorough understanding of your individual situation, we’ll work with you to develop a plan to enhance your wellbeing. Your plan will establish your interests and goals, and identify the services, supports and activities you require to achieve them.

Since our promise is to put your wellbeing at the centre of everything we do, we’ll make sure your plan provides every possible opportunity to make your own decisions and exercise choice and control.

Connect

With your plan in place, we’ll work with you, your family and carers to put you in control of the care and support you receive.

We’ll connect you with the services, supports and activities that will assist you to enhance your wellbeing and achieve the goals identified in your plan.

Assistance provided to guide, support, or undertake on your behalf, activities to ensure you obtain or retain appropriate accommodation

Provision of transport to enable participation in community, social, economic and daily life activities when you are unable to access alternative forms of transport.

Assisting you, your family and support persons to identify behaviours of concern and to design and undertake specific positive support strategies.
